Appointments

         We at Colonial Family Practice realize that each Patient visit is different and is made under different circumstances. You can call in advance to schedule an appointment with fixed time slot and minimal wait times. You can also call in on the day of wanting the visit, such that the schedulers can accommodate you in a time slot that will reduce your wait times. You may also choose to just come on in.

          Please arrive about 10 minutes early for your appointment (new patients should arrive 30 minutes early to their first appointment) in order to complete all necessary paperwork. Please bring your picture ID, insurance card and medication information, including pharmacy location and telephone number with you, as well as any relevant X-rays, diagnostic test results or medical records. Copay, deductible, and coinsurance if applicable are due at the time of service.

          We make every effort to stay on time so your wait will be minimal. From time to time, emergencies may arise which will put us behind schedule. We appreciate your patience during these times.

          If you are unable to keep your scheduled appointment, our office requires at least a 24-hour notification. You may also be assessed a no show charge.
