Important Information About Your Appointment:

Be sure to bring any diagnostic films or other test results that may have been ordered or performed by another physician or any current medications that are currently being taken.

Colonial Family Practice understands that sometimes people suddenly become ill and need to see a physician the same day. For that reason, CFP providers make time to see patients without appointments.

Appointments are preferred to keep patient wait times to a minimum. Due of the number of patients we see, it is important to keep your appointment or cancel at least 24 hours before. There is a fee for no-show appointments, as there are costs associated with scheduling for our practice. Colonial Family Practice does make phone call reminders 24 hours prior to your visit. Please make sure we have your correct number on file.
