At CFP we emphasize disease prevention first. This means that we encourage individuals to have routine checkups, health risk assessments, immunizations, vaccinations, screening tests and personalized counseling on maintaining a healthy lifestyle. These simple steps not only keep you healthy and out of the doctor’s office, but they also decrease your overall healthcare costs.
Even with preventative measures, many individuals will still need to see a family provider for illness, injury or disease. From managing acute and chronic illnesses, to providing relief from the common cold, Colonial Family Practice physicians are experienced in dealing with health issues that are just as diverse and unique as each of our patients. Medicare Annual Wellness Visit
What does Medicare’s new annual wellness visit mean to you? This visit is a great time for the provider and the patient to discuss the patient’s total health and wellbeing. This includes a complete review of the patient’s medications, screening for mental health issues and screening for functional issues.
We will make sure you're up-to-date with all preventive screenings and services, such as cancer screenings and shots. The provider will evaluate and order further tests, depending on your general health and medical history. All screenings and recommendations provided in this visit are based upon Medicare’s initiative to provide preventative services WITHOUT COPAY.
This visit is the perfect time to FOCUS ON PREVENTION to evaluate what the patient and provider need to do to provide the highest level of care possible. The patient will be provided with preventative services checklist with recommendations for the next 5 years.
Patients need to bring ALL MEDICATIONS with them to this visit.
